Registration forms are available in Adobe Acrobat Ž (.pdf) format. The current version of Adobe Acrobat Reader, available for free download at www.adobe.com , will allow you to open, fill out, and print the form.

You can download the required registration and medical forms by clicking on the links below. Please print out the forms, fill them out completely, sign and date the completed forms and mail with your check or money order to the address shown on the registration form, or drop off at the Diamond Community Park during one of the in-person registration dates (please do not bring to the park at other times -- the indoor complex is operated by Plain Township Parks and Recreation and they are not responsible for CSC registration).

Fee Schedule (make checks payable to Community Soccer Club):

	First child in family *	$35.00
	Second child	$30.00
	Third child	$25.00
	Fourth and above	No Charge

* Note: First child is free if parent or guardian is a head coach. Contact the Coaches Coordinator if you are interested in becoming a volunteer coach.
